Donor Choice Registration

The information you provide below will allow us to create an account for you.  This account will generate emails from United Way of Greater Milwaukee that will allow access to donor designation to your agency from our most recent campaign.  If you have more than one Donor Choice contact person at your shop, please fill out the appropriate information and submit the form a second time.

The following information will only be used to establish your access to the United Way of Greater Milwaukee's Donor Online Report Site; your information will not be used/sold for any other purposes.
