Winners of the 2011 Give & Win Sweepstakes announced

United Way of Greater Milwaukee announced the lucky winners of the 2011 Give & Win Sweepstakes during the campaign’s closing celebration held at We Energies on Tuesday, December 13, 2011.

Giving to United Way is truly a win-win! Donors who directed their gift to United Way’s Education, Income and Health portfolio or special initiatives were eligible to win either an iPad / digital cable package; a trip to Costa Rica, complete with spending money; or a brand new 2012 Harley-Davidson® motorcycle and riding lessons. Donors who pledged a new or increased total gift of $50 or more were automatically entered to win a brand new 2012 Ford Focus 5-Door Hatchback donated by Johnson Controls.

This year, 14,034 donors qualified at the $25 level and 10,540 qualified at the $50 level, with the four winners drawn at random.

And the winners are:


Toula Hellauer
, an employee of Badger Mutual Insurance Company, was the winner of this year’s Motorvation prize, a brand new 2012 Ford Focus 5-Door Hatchback donated by Johnson Controls. Bruce MacDonald and Chuck Harvey from Johnson Controls presented Hellauer with her prize.



Jane Eickhoff, an employee of Columbia St Mary’s, was the winner of this year’s Acceleration prize – a brand new 2012 Harley-Davidson® Blackline® Motorcycle and riding lessons. Joanne Bischmann, of Harley-Davidson made the presentation to Eickhoff.


 
Robert Scheidt, an employee of Rockwell Automation, was presented with this year’s Relaxation prize package, which includes a trip to Costa Rica: Airfare from MaxVantage, lodging donated by Rhonda and Tom Cardella, $500 in spending money in the form of a debit card donated by American Express, and a $500 pre-trip shopping spree in the form of a gift card for use at any Boston Store. Mischelle Peterson from MaxVantage and Gail Monreal from Bon Ton, Inc. and the Cardellas made the presentation to Scheidt.



Brandon Gray, an employee of Time Warner Cable, was the lucky winner of this year’s Innovation prize package, which includes an Apple iPad from Infoblox, one year of Time Warner hi-speed broadband, one year of digital cable and a cable iPad app from Time Warner Cable. Britt Allen from Infoblox and Tom Adams from Time Warner Cable, presented the package to Gray.

GIVE & WIN Sweepstakes Incentive Rules for 2011 Campaign

Sponsored by United Way of Greater Milwaukee, 225 W. Vine Street, Milwaukee, WI 53212.

1. This incentive promotion begins on September 1, 2011, and ends at 5 p.m. Central Time on November 18, 2011. Participants must be legal residents of Wisconsin, at least 18 years of age and possess a valid driver’s license.

2. The winners will receive, in the order drawn, the following prizes donated to United Way of Greater Milwaukee by the respective companies:

A. 2012 Ford Focus 5-Door Hatchback donated by Johnson Controls
B. 2012 Harley-Davidson® Blackline motorcycle and riding lessons
C. Trip to Costa Rica: Airfare, lodging, $500 spending money in the form of an American Express debit card and a $500 pre-trip shopping spree in the form of a gift card for use at any Bon Ton Store including Boston Store
D. Apple iPad, 1 year of Time Warner hi-speed broadband, 1 Year of Time Warner digital cable and cable iPad app

Only one prize per household can be awarded. Odds of winning depend on number of eligible entries received.

3. To qualify, a United Way of Greater Milwaukee donor must timely increase both his/her total 2010 Campaign gift by $50 or more, while increasing his/her designation to United Way of Greater Milwaukee’s Education, Income or Health Portfolio or special initiatives. This will automatically enter the donor for a chance to win prize A.

Entry without donation can also be made by sending a letter to United Way of Greater Milwaukee, Attn: Drawing, 225 W. Vine Street, Milwaukee, WI 53212, containing name, address and phone number. Only one entry, automatic or otherwise, per person.

4. The Pledge Form or Entry Letter must be returned to United Way of Greater Milwaukee by 5 p.m. Central Time on November 18, 2011. United Way of Greater Milwaukee will not be responsible for companies that fail to submit their employees’ pledge forms by this date.

5. The winners of the sweepstakes will be drawn randomly and announced at the Campaign Closing Celebration on December 13, 2011.

6. Employees of United Way of Greater Milwaukee and members of their immediate families are not eligible to participate.

7. The winner is required to provide proper photo identification, valid driver’s license for motorcycle and automobile winners and verification of compliance with these rules upon presentation of all prizes.

8. By entering, all participants hereby agree to comply with these Rules, represent that he/she is eligible, and give consent to the use of his/her name and/or photograph for publicity purposes without further compensation, unless prohibited by law. All entrants release United Way of Greater Milwaukee, Bon-Ton Stores, Harley-Davidson Motor Company, Tom and Rhonda Cardella, Infoblox, Johnson Controls, American Express, Maritz, Time Warner Cable and their employees, directors, officers and agents, parent, subsidiary and affiliated companies, advertising, fulfillment, publicity and promotion agencies from any and all claims or liability in connection with their participation in this promotion. Entrants agree to hold harmless United Way of Greater Milwaukee, Bon-Ton Stores, Harley-Davidson Motor Company, Tom and Rhonda Cardella, Infoblox, Johnson Controls, American Express, Maritz, Time Warner Cable and their parents, subsidiaries, affiliates, agencies, officers, directors and employees from responsibility for damages, losses or injuries, including death, personal injury or property damage, due in whole or part, directly or indirectly, from participation in this promotion or acceptance, possession, use or misuse of the prize.

9. No substitution or transfer of prize or redemption for cash equivalent, except by United Way of Greater Milwaukee due to unavailability, in which case a prize of equal or greater value will be substituted. United Way of Greater Milwaukee reserves the right to cancel, terminate or modify this promotion if not capable of completion as planned, and to award the prizes as of that date as provided above.

10. Costa Rica trip winner must book and utilize airfare and lodging by December 31, 2012. Failure to do so will result in a forfeit of the prize. Airfare and lodging is subject to availability when booking. Prize value of airfare shall not exceed $2,000 ($1,000 per person, up to two tickets). Any additional tickets or costs above this amount will be incurred by the prize winner.

11. Motorcycle and automobile winners may be required to pay any title transfer costs and other applicable taxes and costs relating to the transactions, including, but not limited to, federal and state income taxes.

12. Participation in this promotion may affect the tax deductibility of your gift. You should consult your tax advisor for more detailed information.

13. The name of the prize winners will be available after December 13, 2011 at

Prize winners must accept terms and conditions of the prize within ten days of notification of winning. Notification will take place via phone call, email and/or certified letter. Automobile, motorcycle and iPad winner must take ownership of item by January 31, 2012.

Ensuring the Integrity of the Sweepstakes
Those individuals that qualify for the Sweepstakes are automatically entered into the drawing as their pledges are processed. Companies, which do not typically provide individual pledge information, may submit a list of those employees who have qualified. Those names are added to the database of automatic qualifiers. Additionally, those people who submitted an entry outside of the automatic qualification process are also entered into the database. Although we cannot publish the list of all those who qualify, if a workplace campaign requests a list of qualifiers from their own company, United Way of Greater Milwaukee can provide it prior to the drawing, but after all pledges received by November 18, 2011 have been posted.
